Dear Student,

My name is Raquel Villareal, I am a Registered Nurse and also your Clinical Facilitator for this course.  I teach the practical component of the course. You will attend Clinical Workshops in our simulated environment prior to your placement, where we will practice a set of skills for assessment. This includes grooming, nutrition and hydration, transfer and mobility, maintaining healthy body system, and other tasks involve as a future assistant in nursing. 

Two months in advance, you will be asked to submit paperwork requirements. This will be sent to the nursing home or the healthcare facility where you will do your 120 hours of clinical placement. It include the following :

        1. Photocopy of your school I.D.

        2. Statutory Declaration, signed by Justice of Peace

        3. Police Clearance - (choose STUDENT)

        4.  First Aid Certificate

        5.  Copy of your vaccination

        6.  Copy of your visa (some nursing home requires copy of your visa)

        7.   Copy of your recent blood test

Please bring the original of the above requirements and a photocopy.  I will certify that I have seen the original before you upload to the Moodle site as Part 1 of your requirement with a stamp. 

A confirmation letter will be sent to you upon approval of the accepting nursing home and to give you further details of the Clinical Placement.
